Gift Card Program

Health Services for Children with Special Needs, Inc. (HSCSN) would like to help you and your family take an active role in your health. We have teamed up with OTC “Over the Counter” Network, a global company that offers rewards for taking care of your health, to announce HSCSN’s Wellness Gift Card Program. 

What is the HSCSN Wellness Gift Card Program?

Starting July 1, 2023, HSCSN enrollees will be able to earn up to $75 a year for completing wellness goals. These goals are related to the Early and Periodic Screening, Diagnostic and Treatment (EPSDT) program. A HSCSN Care Manager can help with completing these goals through your care coordination plan. When a goal is completed, money will be placed on your gift card to use at local participating stores.

How do I sign up?

All HSCSN enrollees will automatically receive a reloadable gift card in the mail. Instructions on how to turn your gift card on and use it at local stores will be included in the letter.

How is money placed on the gift card?

Money will be loaded to the gift card as soon as HSCSN confirms a wellness goal has been completed. It could take up to 30 days to confirm a goal has been completed.

How are wellness goals completed?

There are four wellness goals to complete each year to receive your incentives:

  1. $15 for adult or child well visit (preventive service appointment)
  2. $15 for one dental preventive appointment
  3. $15 for annual visit with HSCSN Care Manager (virtual included)
  4. Up to $30 for completing two clinical goals you identify with your Care Manager related to your health needs

Will HSCSN Wellness Gift Card funds expire?

Yes, the money you earn for completing your wellness goals should be used within one year of completing the goal. If you request a new card or receive a new card, unused funds will transfer from your old card. They will not expire if you have not activated your card.

How can I buy items? 

You can use this gift card at several local stores. These stores include Giant, Walmart, CVS, Dollar General, and Walgreens.
