Your HSCSN Care Coordination Team is always as close as your HSCSN EASY Connect mobile application. Your larger Care Team includes the HSCSN Care Manager, Care Support Specialists, the Primary Care Physician (PCP), the Primary Dental Provider (PDP), and any other healthcare providers the enrollee sees. This app connects enrollees and caregivers to the HSCSN Care Coordination Team. They want to help you feel the best you can. Your care team can help you manage your health concerns. They can even connect you to resources and in-person support. We are connecting you with tools and capabilities to manage your health.

With EASY Connect you connect to your care team and the resources you need when you need them. This includes Nursing Triage and Urgent Telehealth.
