HSCSN enrollees get:
HSCSN’s Family and Community Development Outreach team helps enrollees and their families improve their health and social needs. FCD collaborates with more than 200 community partners, government agencies, and the local school system to increase awareness of HSCSN benefits, especially as they relate to housing, nutrition, school engagement, and transition resources. Together, these partnerships enable HSCSN to deliver a comprehensive array of services.
