Family and Community Development Outreach Department (FCD)

At HSCSN, we know your health and well-being is about more than just health care. As a result, the FCD team connects our enrollees to services and programs in the community. The Outreach Team:

  • Plans community events to address the needs of our enrollees and their families.
  • Host wellness programs and support groups.
  • Makes sure people who qualify for HSCSN benefits are aware of them. To qualify, you must:

Community Connections 

HSCSN’s Family and Community Development Outreach team helps enrollees and their families improve their health and social needs. FCD collaborates with more than 200 community partners, government agencies, and the local school system to increase awareness of HSCSN benefits, especially as they relate to housing, nutrition, school engagement, and transition resources. Together, these partnerships enable HSCSN to deliver a comprehensive array of services.
